A "sign-off round" is one briefing that has to be evidenced: a toolbox talk, an allergen change, a site rule, a recall notice. Time one round properly before you trust the answer.
Both of these are blank on purpose. We do not have a credible published figure for either, and a made-up one would make this whole page worthless. Use your own, from your insurer, your claims history or your HSEQ team. Leave them blank and the panel below stays quiet.
Nothing here is hidden and nothing is weighted. If you disagree with an assumption, change the input it comes from; there are no coefficients you cannot see.
